The incident occurred around 2:17 p.m. at the site located at 3000 Grace Avenue, prompting a massive response from city emergency services.

Rescue Efforts and Recovery Operations

First responders arrived on the scene immediately after the collapse. Through initial surface searches, three individuals were rescued. One person was critically injured and transported to Penn Presbyterian Medical Center, where they later died from their injuries. Two other individuals were treated at the scene and released.

Mayor Cherelle L. The general contractor for the project is HSC Builders and Construction. The project involved the new construction of a seven-story parking garage with ground-floor retail space. The owner of the garage is the Children’s Hospital of Philadelphia (CHOP).

Inspections and Investigations

City officials confirmed that eight permits were required for the project and were properly issued. All inspections were reportedly up to date at the time of the collapse. Licenses and Inspections Commissioner Basil Morenda stated that the structural collapse was contained to the stair tower area.

However, the pre-cast concrete segments were manufactured offsite and installed by the manufacturer. This process requires special inspections, not standard ones. These inspections were assigned to Valerie Moody of GAI Construction Monitoring Services.

The Occupational Safety and Health Administration (OSHA) and the Philadelphia Department of Licenses and Inspections (LNI) have initiated investigations into the incident. The LNI’s audits and investigations unit will examine all contractors and subcontractors involved. The site remains an active emergency scene under the control of the Philadelphia Fire Department.
